Q&A how to code for Lumbar DDD when documented with lumbar spinal stenosis/radiculitis?

Hello,

Can you please tell me how to go about coding for displacement/degeneration of intervertebral discs/spondylosis when documented along with spinal stenosis? In ICD 9 723 and 724 (stenosis codes) excludes 721/722 codes. I have found cases where the doc states spondylosis/DDD along with stenosis/radiculitis and it has become very confusing.
Do we code for only one or both ?
